Membership Application

To apply for membership with PSN, you will need submit an application form.

You can also obtain a form by contacting PSN headquarters and requesting one to be mailed or faxed to you. Please follow the instructions below while filling out the form and then send the form to PSN.

1.	Print and fill out the application form fields as completely as possible.
2.	Select the appropriate option relative to a new or renewal membership.
3.	For renewal memberships, please include your PSN member number.
4.	Select the appropriate membership level.
5.	Print the application form from your browser.
6.	Sign the application.
7.	Write a check or money order in the amount appropriate for the selected membership level and make it payable to: World Chicago Stepping Assn Association.
8.	Mail your signed application with your payment to Chicago Steppin' Association, P.O. Box 816, Pacifica, CA 94044-0816. If you have questions about membership, please e-mail PSN Secretary
